Custom Cake Decorator Appointment Form Custom Logo Notepad

This notepad has preprinted forms to use when you speak to a new customer to make an appointment and gather information about their event. Fill these out to take to the tasting or planning appointment so that you have the basic information that the customer sent you or told you over the phone. You can leave the forms attached to the notepad or tear them off to keep them in a customer file. These were the same intake forms that I used when I did custom wedding cakes.
