Medical Excuse Black and White Doctor's Notes

This is a template for a doctor's note that excuses the patient for the time of the patient's appointment. You can edit the text to show the name of the doctor or medical practice in a bold font. There's non-bold text for the office address and phone number. Also, blank lines are available to write the patient's name and appointment date and time. All text is black and can be edited or deleted. The notes in this notepad are white.
