Medical Excuse for Patient Doctor's Note Template

This is a template for a doctor's note that excuses the patient for the time of the patient's appointment. You can edit the text to show the name of the doctor or medical practice in a bold font. There's non-bold text for the office address and phone number. Also, blank lines are available to write the patient's name and appointment date and time. Finally, there's a place to write the dates the patient is excused from work or school. All text is black and can be edited or deleted. The notes are white.
